“Abnormal Liver Blood Tests? What It Means and When to See a Specialist”

Abnormal liver blood tests are one of the most common reasons patients are referred to gastroenterology clinics.

In many cases, the cause is mild and reversible — but sometimes it can indicate an underlying liver condition that needs proper assessment.

The key is knowing when to reassure and when to investigate further.


What Do “Abnormal Liver Tests” Actually Mean?


Blood tests such as:

  • ALT

  • AST

  • ALP

  • Bilirubin

can become abnormal for many reasons — not all of them serious.

Common causes include:

  • Fatty liver disease (very common)

  • Alcohol-related liver injury

  • Medications or supplements

  • Viral hepatitis

  • Less commonly, autoimmune or metabolic conditions

  • Blood tests are repeated multiple times

  • No clear diagnosis is made

  • Patients are left unsure whether to worry

This leads to unnecessary anxiety — or sometimes delayed diagnosis.

  • Liver tests remain abnormal over time

  • Levels are significantly raised

  • You have risk factors (weight, diabetes, alcohol, family history)

  • You have symptoms such as fatigue, jaundice, or abdominal discomfort

  • Detailed history (including alcohol, medications, metabolic risk)

  • Targeted blood tests

  • FibroScan (non-invasive liver scan where appropriate)

  • Imaging if needed

The aim is to:

The Most Common Cause: Fatty Liver Disease


Fatty liver is now the leading cause of abnormal liver tests in the UK.

The good news:

👉 It is often reversible if identified early

But:

👉 In some patients, it can progress to fibrosis or cirrhosis

Which is why proper assessment matters.

Most abnormal liver tests are not dangerous in the short term, but they should not be ignored.
